  • Exclusive: Inheritance and Clinical Practice of Traditional Chinese Medicine

    In response to the "Traditional Chinese Medicine Development Plan" of the Fourteenth Five Year Development Plan, and to strengthen the departmental development of traditional Chinese medicine and clinical research, this paper systematically analyzes the scientific research progress in the inheritance and innovation of Chinese medicine in the past 20 years. The contents, include the research progress in clinical registration of traditional Chinese medicine at home and abroad in the past 20 years, the clinical research progress in the treatment of infectious diseases and chronic non infectious diseases, research and application progress of standardization of traditional Chinese medicine, research and application progress of new technologies in traditional Chinese medicine, and the "the Belt and Road Initiative" promoting the development of clinical research of traditional Chinese medicine in China. The results show that the clinical research and innovation achievements of Chinese medicine in the past 20 years have provided an important scientific and technological guarantee for inheritance, innovation and development of Chinese medicine. Chinese medicine has historically entered the mainstream medical system in the world and its international influence is significantly improving.

    Patients with acute severe heart failure treated in the emergency department of Chuiyangliu Hospital from July 2019 to July 2020 were taken as the research subjects in this retrospective parallel controlled clinical study to compare the efficacy and prognosis between the standard western medicine treatment (control group: spironolactone+captopril) and integrated traditional Chinese and western medicine treatment (integrated group: spironolactone+captopril+Shengmai Decoction). The cardiac ejection function was measured by echocardiography. The results showed that the E/A and LVEF of the integrated group were higher than those of the control group, while the LVES and LVED were lower than those of the control group; the incidence of adverse reactions such as hypokalemia, cough, and hyperkalemia in the integrated group was lower than that in the control group; the QOL score in the integrated group was higher than that in the control group. This suggests that Shengmaiyin combined with western medicine has certain advantage in the treatment of acute severe heart failure, which can further improve cardiac function, enhance left ventricular ejection ability, reduce adverse reactions and improve the prognosis and quality of life of patients.

    Acupuncture and non-steroidal anti-inflammatory drugs (NSAIDs) are common conservative treatment methods for scapulohumeral periarthritis. This study was a prospective non-RCT and patients with scapulohumeral periarthritis were divided into the control group (orally intake of celecoxib) and acupuncture group (acupuncture at the points of acromion, acromion and anterior shoulder), and the patients in each group were further divided into three subgroups of mild, moderate and severe pain according to visual analog scale (VAS). This study aimed to compare the efficacies of the two treatments on scapulohumeral periarthritis with different pain degrees. Indicators included the VAS, Constant shoulder score, range of motion (ROM), etc. The results showed that in patients with mild and moderate pain, there was no significant difference in VAS or Constant scores between the two groups, but the ROM of the acupuncture group was more improved. In patients with severe pain, the VAS of the acupuncture group was higher than that of the control group, and its Constant score was lower than that of the control group, no difference of ROM was found between the two groups. This study suggests that the acupuncture treatment acquires a better efficacy in patients with scapulohumeral periarthritis who had a mild to moderate pain, while NSAIDs acquires a better efficacy in patients with severe pain.

    Primary dysmenorrhea (PD) is the most common gynecological disease in women of childbearing age. Xiaoyao powder is a classical traditional Chinese medicine prescription for the treatment of PD. In order to clarify the effect of Xiaoyao powder on the endocrine level and the potential mechanism of efficacy, PD patients were included in the present study, and the changes of female hormone level and uterine morphology before and after treatment were assessed and compared. The morphological changes of the uterus were measured by color Doppler technique, the serum endocrine indicators included the serum estradiol (E2), progesterone (P), etc., and the efficacy indicators included the visual analog scale (VAS) of pain and the Cox menstrual symptom scale (CMSS). The results found that the serum E2 level, uterine anterior-posterior diameter, endometrial thickness, VAS, and CMSS were significantly lower than those before the treatment, while the serum P level was significantly higher . There was a positive correlation between serum E2 and uterine anterior-posterior diameter, and a negative correlation between serum P and endometrial thickness, correlations between serum P, E2 and VAS, CMSS were also found. It suggests that Xiaoyao Powder has a definite effect on PD, which can reduce the uterine anterior-posterior diameter and endometrial thickness of PD patients, and improve the symptoms and duration of dysmenorrhea by adjusting E2 and P levels.

    Sepsis is a difficult problem in the field of acute and critical illness. The heart is the most susceptible organ in the pathogenesis of sepsis. Cardiac function injury and coagulation disorder are common complication in patients with severe sepsis. COVID-19 can cause viral sepsis, often accompanied by coagulation disorders and myocardial damage. This paper introduces the pathogenesis of traditional Chinese medicine (TCM) of sepsis and the pathophysiology of western medicine, and the significance of coagulation disorder in the formation of sepsis is analyzed. Through discussing the treatment means of traditional Chinese and western medicines it can be seen that TCM has certain advantages in infectious diseases and coagulation dysfunction, and can supplement the deficiency of western medicine treatment to a certain extent. However, further systematic studies are still needed to clarify the effectiveness of TCM treatment.

    To evaluate the therapeutic effect of Mongolian traditional prescriptions on patients with chronic obstructive pulmonary disease (COPD), a total of 153 COPD patients at stable stage were recruited, among which 94 patients were treated with budesonide, formoterol or indacaterol (western medicine group), and 59 patients were given additional Agari, Zandan-20, Qingfeirunze pill, etc. (Mongolian medicine group). After three months treatment, there was no significant difference in the pulmonary function indexes or six-minute walk test (6MWT) results between Mongolian and western medicine groups. There were significant differences in symptom score of Saint George's respiratory questionnaire (SGRQ) between the two groups (Adjusted P= 0.021). Compared with the baseline, the chronic obstructive pulmonary disease assessment test (CAT) score of the Mongolian medicine group decreased by 6 points, which was significantly higher than that of the western medicine group (Adjusted P< 0.001). For the CAT score, the proportions of patients decreased by 2 points or more, and the ratio of treated patients with lower or constant CAT score in the Mongolian medicine group were significantly higher than those in the western medicine group (P△CAT≤-2=0.001, corrected Pdecreased/fixed number=0.035). The disease impact score of SQGR decreased by 11.19 points in the Mongolian medicine group and 6.98 points in the western medicine group. The score of the Mongolian medicine group was significantly higher than that in the western medicine group (P=0.014). It was confirmed that the Mongolian medicine combined prescription can significantly improve respiratory symptoms, elevating the cardiopulmonary function.

    In order to identify the efficacy of Xiaoer Jianpi Bufei ointment on the treatment of recurrent respiratory tract infection (RRTI) in children and its potential immunology mechanism, children with RRTI admitted to Foshan Fuxing Chancheng hospital from Jan 2019 to Dec 2020 were randomly divided into the combination group of traditional Chinese medicine (TCM) and western medicine (Xiaoer Jianpi Bufei ointment + immunoactive peptide) and control group (immunoactive peptide). Indicators included: the peripheral blood CD3+, CD4+ and CD8+ detected by flow cytometry, maximum expiratory flow (PEF), and TCM syndrome score for RRTI. Subjects were followed up to 1 year after the treatment. The results showed that after the treatment, the levels of CD3+ and CD4+ , PEF, RRTI and TCM syndrome scores of the combination group were significantly higher than those of the control group. During the treatment, the adverse reactions (2.22%) of the combination group were lower than those of the control group (13.33%), and finally the efficacy of the combination group (97.77%) was higher than that of the control group (84.44%). It suggests that the integrated Chinese and western medicines of Xiaoer Jianpi Bufei ointment combined with immunoactive peptide for treatment of children's RRTI can further improve the immune level and expiratory function, and relieve the clinical symptoms, which may improve the clinical safety and efficacy.

    In order to estimate the efficacy of Buzhong Yiqi Decoction combined with biological mesh on the treatment of inguinal hernia, a clinical trial and follow-up study was designed with inguinal hernia patients as the subjects, who were randomly divided into the polypropylene mesh group, biological mesh group, and combined group (biological mesh + Buzhong Yiqi Decoction). The indicators included Perioperative indexes, visual analogue scale (VAS) for pain, as well as the incidences of complication, recurrence, and re-operation at 1-year follow-up. The results showed that the hospital stay, post-operative VAS at 1 d, 3 d and 7 d of the combined group were significantly shorter than those of the other two groups; 1-year follow-up showed that the incidences of chronic pain, foreign body sensation, total complication, and recurrence of the biological mesh group and the combined group were significantly lower than those of the polypropylene group. It suggests that Buzhong Yiqi Decoction combined with biological mesh on treatment of inguinal hernia has superior safety and benefit on post-operative rehabilitation, which can decrease the post-operative pain and hospital stay.

    Traumatic hemorrhage is an important challenge in the field of first aid and may cause imbalance of the coagulationanticoagulation system. Yunnan Baiyao Baoxianzi is a classic first-aid medicine in the treatment of traumatic hemorrhage in traditional Chinese medicine. It has the effects of both hemostasis and anticoagulation. In this study, orthopaedic trauma patients at the emergency center of Hohhot First Hospital from Jan 2018 to Dec 2021 were taken as the research objects. Through a prospective parallel controlled clinical study, the hemostatic efficacy of baozi was systematically evaluated in terms of intraoperative blood loss, postoperative drainage, hemoglobin decline, prothrombin time (PT), activated partial prothrombin time, fibrinogen (FBG), blood transfusion rate, prognosis of bleeding/coagulation adverse reactions, etc. The results showed that the intraoperative bleeding volume, postoperative drainage volume, hemoglobin decline, blood transfusion rate, PT, and prognosis of bleeding/coagulation adverse reactions in the Baoxianzi group (n=27) were significantly lower than those in the control group (n= 24) and the FBG in the Baoxianzi group was significantly higher than that in the control group. Thus, Baoxianzi can maintain the balance of coagulation-anticoagulation system, significantly reduce bleeding volume and benefit anticoagulation at the same time, preventing TIC and reducing adverse prognosis reactions.

    In order to determine the efficacy and prognosis of procedure for prolapse and hemorrhoids (PPH) combined with Buzhong Yiqi Decoction in the treatment of severe mixed hemorrhoids, a prospective parallel controlled clinical study was conducted. Patients with severe mixed hemorrhoids diagnosed and treated in our hospital from Feb 2019 to Jan 2021 were taken as the research objects, and they were successively included in the control group (PPH) and the combined group (PPH + Buzhong Yiqi Decoction). Visual analog scale (VAS) was used to evaluate the pain degree of postoperative wound, record the short-term complications during hospitalization, record the long-term complications and recurrence of hemorrhoid prolapse during one-year follow-up after operation, and evaluate the anal function of patients' prognosis by anal distension score. The results found that postoperative VAS of the combined group was significantly lower than that of the control group (P<0.001). The one-year follow-up results showed that the anal distension score, long-term complication rate, and recurrence rate of hemorrhoid prolapse of the combined group were significantly lower than those of the control group (P=0.002) (P=0.014) (P=0.009). This study suggests that the curative effect and prognosis of PPH combined with Buzhong Yiqi Decoction in the treatment of severe mixed hemorrhoids are better than those of PPH alone. The combined treatment can effectively reduce the postoperative wound pain, improve the anal function of patients' prognosis, and reduce the incidence of long-term complications and the recurrence rate of hemorrhoid prolapse.

    Frankincense can relax sinew, promote flow of qi and blood circulation, and remove blood stasis. It is a commonly used medicinal material in traditional Chinese medicine for the treatment of arthralgia, dampness blocking disease and others. Frankincense drugs have the risk of inducing adverse reactions. This study used the traditional Chinese medicine prescriptions containing frankincense drugs in Huguosi traditional Chinese Medicine Hospital Affiliated to Beijing University of traditional Chinese Medicine from Jan to Mar 2022 as the research object. This retrospective study aimed to clarify the independent factors that affect the adverse reactions of frankincense through statistical description, correlation analysis and multivariate regression analysis. Indicators included dosage of frankincense in the prescription, mode of administration, as well as patient's age, gender, allergy history, adverse reactions, etc. Altogether 108 prescriptions were received, of which 7 prescriptions caused adverse reactions, including 6 skin allergies and 1 diarrhea. Multivariate regression analysis showed that the patient's allergy history was the only independent risk factor affecting the adverse reactions of frankincense drugs. It is shown that the adverse reactions of frankincense formula used in the clinical practice of traditional Chinese medicine at this stage are only related to the patient's allergy history, and which should be the focus of rational drug use and adverse reaction control.
